Supporting Schools in Strengthening Transition Planning
Transition planning is a required part of special education, but many schools and programs are asking important questions:
- Are students receiving meaningful transition services?
- Are transition assessments driving planning and decision-making?
- Do IEPs connect current programming to life after high school?
- Are students developing the skills they will need for adult life?
- Is our program preparing students for successful post-school outcomes?
Addressing these questions can be challenging when teams are balancing compliance requirements, competing priorities, and the diverse needs of students.
Connecting Transition Planning to Student Outcomes
Transition planning should be more than a checklist. It should help students build the skills, experiences, and supports they need after graduation.
I partner with school districts, collaborative programs, private schools, and educational teams seeking additional support with transition planning, assessment, implementation, and program development.
Together, we examine current practices, identify areas of strength and growth, and explore ways to better align educational programming with students’ future goals.
